This policy describes information handled through this website. It does not describe a treating healthcare provider's handling of medical records or replace any privacy notice that a provider gives you. The website provides information about SoLá Pelvic Therapy; it is not an emergency service or a substitute for medical care.

The Patient Inquiry form asks for your first and last name, email address, phone number, country or region, street address, city and postal code. It also offers phone, text and email contact preferences. Information you send through a separate email or other communication may include the contents of that communication.
Please do not send medical records, detailed symptoms, insurance information or other sensitive health information through general website inquiries or ordinary email. Ask your treating provider how to communicate securely about your care.
How inquiry information is used
We use inquiry information within SoLá Pelvic Therapy to respond to your request. The Patient Inquiry form is configured with spam filtering and reCAPTCHA, and a submission-summary email notification to the site Owner collaborator role. The website also includes an embedded Google Maps feature. These services support the operation of the website and its features.

Inquiries are handled within SoLá Pelvic Therapy rather than sent to outside healthcare providers. Website and communications service providers process information to support the services described above, including Wix hosting and forms. Keeping inquiries within SoLá Pelvic Therapy does not mean that no service provider processes information.
Retention and protection of information
We retain inquiry information indefinitely unless it is deleted. Information may be deleted without notice, subject to applicable legal obligations and privacy rights. This is not a promise that any individual inquiry will remain available permanently.
Copies of inquiries are stored on the SoLá Server.
No internet transmission or storage system can be guaranteed completely secure.
Your choices and requests
